The Lineup: What is Being Tested?
Total & Free Testosterone
What it is: Testosterone is the primary male sex hormone. Total Testosterone is the entire amount floating around in your blood. Free Testosterone is the tiny fraction (usually only 1–2%) that is actually active, unattached, and ready for your body to use.
Estradiol
What it is: The primary form of estrogen (the "female" hormone), but men need it too! Men naturally convert a portion of their testosterone into estradiol.
SHBG (Sex Hormone-Binding Globulin)
What it is: A protein made by your liver that acts like a "hormone sponge." It travels through your blood, grabs onto testosterone, and locks it up.
LH & FSH (Luteinizing Hormone & Follicle-Stimulating Hormone)
What they are: Brain signals. Your brain (specifically the pituitary gland) releases these hormones to talk to your testicles. LH tells your testicles to produce testosterone. FSH tells your testicles to produce sperm.
DHEA-S (Dehydroepiandrosterone Sulfate)
What it is: A precursor hormone made by your adrenal glands (your stress centers). Think of it as the raw ingredient your body uses to create other hormones, including testosterone and estrogen.
Cortisol (4-Point Salivary)
What it is: The primary stress hormone. Unlike the other tests which use blood, this one uses saliva samples taken four times throughout a single day (morning, noon, evening, and night).
Add More to the Lineup?
Metabolic & Insulin Dynamics (Fasting Insulin + HbA1c): Visceral belly fat produces aromatase, the enzyme that turns precious testosterone into estrogen. High fasting insulin blocks the testes from making T.
The Complete Lipid Particle Profile (ApoB + lp(a)): Forget standard Cholesterol/LDL. Apolipoprotein B (ApoB) is the gold standard metric for cardiovascular risk. Since testosterone optimization (even natural) impacts lipid metabolism, you must baseline and monitor ApoB to ensure the cardiovascular highway is clear.
Advanced Inflammation (hs-CRP + Ferritin): Chronic systemic inflammation ("inflammaging") directly destroys Leydig cell function in the testes. If hs-CRP is elevated, the first protocol is reducing inflammation before testosterone-boosting herbs.
Liver Function & Detoxification (ALT, AST, GGT): Hormones are metabolized and bound in the liver. A sluggish, fatty liver leads to skewed SHBG and poor hormone clearance.
DunedinPACE (Pace of Aging Speedometer)
Also included in advanced epigenetic panels. It tells you exactly how fast a man's cells are degrading right now. If his pace is 1.2, he's aging 20% faster than calendar time. Gauging lifestyle protocols with science.
TruDiagnostic TruAge Complete (The Ultimate Epigenetic Clock):
This is the modern pinnacle of longevity data. It doesn't just calculate biological age; it features the SymphonyAge algorithm (developed with Harvard), which breaks down the biological age of 11 specific organ systems, including an explicit "Endocrine/Hormone Age" and "Inflammation Age."
Advanced Mitochondrial & Organic Acids Testing (OAT):
Testosterone synthesis happens inside the mitochondria of the Leydig cells. New advanced functional panels map mitochondrial energy production, coenzyme Q10 status, and oxidative stress markers.
